
WELL-CHILD EVALUATION
The practice of preventative medicine is central to Pediatrics. Periodic well visits enable us to review the immunization status of your child and to become familiar with his or her individual pattern of growth and development. We require initial history and physical examinations when first joining the practice and request regular examinations after that.
If your child makes yearly visits after the age of two, we are then able to sign his or her school, camp and work forms as they appear. If no well examination has been performed in the last year, we can not sign the form.
During adolescents' well visits, we allow time for discussion of personal health and safety issues confidentially with the patient, and provide separate parent conferences. The parent conferences are done up to & including the college physical.
Interscholastic athletes (grades 6-12) should be aware that their physical examinations must be done within one year of the sport beginning. Once the exam is done, they do not need to repeat it for each sport.
You should check with your child's school or camp for any other requirements they might have.
